  • Step 2- Independent Contractor Agreement

  • This letter shall constitute a contract between SJL, Inc. (SJL), and you (contractor).

    1. I (contractor) agree to engage the services of SJL to be my manager/agent, though not exclusive, for the term of this agreement.
    2. I agree for SJL to collect and receive monies on my behalf and to retain all sums due to SJL at any time. I also authorize SJL to approve and permit the use of my name, photograph, likeness, and voice on my behalf and upon my approval. This authorization shall include promotion for SJL, including, but not limited to photographs, books, electronic media, including, but not limited to computer on-line services, the Internet and CD-ROM. I hereby assign to SJL the proceeds of all assignment, against which advance payment has been made. Cancellation fees, expenses, foreign billings, advertising agencies, and other clients as determined by a SJL representative are paid to artist when payment is received by manager/ agent unless other agreements are made prior to assignment. Manager/agent will take all responsible steps to collect the amounts due with respect to the thereto.
    3. I acknowledge, I am an independent contractor, and will perform work on behalf of SJL as an independent contractor. I agree I will be responsible for all taxes, self-employment taxes, insurance or other governmental obligations.
    4. As and independent contractor, I agree I am responsible for all travel and release SJL from all liability, indemnity and subrogation for any and all claims, damages, injuries or other liabilities incurred to person or property before, during and after an SJL assignment, including injuries and damages during travel.
    5. I agree the term of this agreement shall be for a period of one (3) years commencing as of this date, may be cancelled by either party without penalty with written notice.

  • An independent contractor is defined by law as one who engages to perform certain services for another, according to his own manner, method, free from control and direction of his contractor in all matters connected with the performance of the service, except as to the result or product of the work.

    Below are statements to help you decide if you are an independent contractor. No one statement is controlling, and your status is based on all the facts in your situation. If a statement describes your situation, then check the box. If the six statements* below do not describe your business, you should not sign the Exempt Status Under the Workers’ Compensation Act.

    12—A middleman known in the investment community as a nominee or custodian

  • Exemption from FATCA reporting code. The following codes identify payees that are exempt from reporting under FATCA. These codes apply to persons submitting this form for accounts maintained outside of the United States by certain foreign financial institutions. Therefore, if you are only submitting this form for an account you hold in the United States, you may leave this field blank. Consult with the person requesting this form if you are uncertain if the financial institution is subject to these requirements.

    A—An organization exempt from tax under section 501(a) or any individual retirement plan as defined in section 7701(a)(37)
